ABOUT ME:
I’m Alissa Wolfe.

I’m a doctor of physical therapy and chronic pain specialist. I help people who are fed up and frustrated with chronic pain retrain their nervous system so they can beat chronic pain and get their life back.

In other words, if you have chronic pain and are looking for a new and improved way to manage your pain with an approach based on neuroscience, I’m your girl.

I got out of the clinic and started my online chronic pain coaching business so I could provide that nervous system retraining piece that is missing from pain management in the health care system.

Empowering people with chronic pain is my calling. I wasn’t okay knowing that my patient’s with chronic pain had no one to turn to, no one who would help, and no one who would believe them.

I decided I would be that person, even if in the beginning I didn’t know everything I needed to know to help them. I pushed myself to learn and I discovered that we were trying to treat the “tissue issue” when people with chronic pain have a nervous system “pain problem”.

Changes in the nervous system were contributing to pain and yet no one was addressing those changes.

By adding this element of neuroscience and nervous system retraining to my treatment approach, clients and patients are getting better than ever.

I am having some health struggles and this video shows a little bit of what a rough day looks like for me. I have had and will have worse days in the future but this is probably the only video where I will focus on that. I am having to use my wheelchair full time and we don’t yet know the cause of the problem with my leg.

“I’d been dealing with intense chronic back pain for 25 years and was at the end of my rope. Trips to the ED, physical therapy and consultations with orthopaedic and neurosurgeons hadn’t helped. I was advised to see surgeons in Boston or at Dartmouth-Hitchcock in Hanover. I chose Dartmouth-Hitchcock. Dr. Lollis told me that surgery wouldn’t help me but encouraged me to try Functional Restoration Program. The three week program was really hard in the beginning; I actually thought about leaving. I love exercise but was afraid of what they were having us do. But the staff found a way to get us moving. By the second week I was in a nice rhythm and the class discussions during the third week helped us think through how we’d keep our program going at home. It was wonderful to work through the program with other people. Doing therapy on my own had been very isolating and never really clicked for me. And everyone on the program staff was so dedicated. Every day was filled with activities, comments, and energy that you don’t find very often anywhere. Now, one year out, I’m back to working 30 — 40 hours a week in the heating and air conditioning business. I’ve gone on back to back 8 — 12 mile hikes and took an 8 hour kayaking trip.”

As the damage of opioids becomes clear, doctors are increasingly looking for alternatives for managing pain. Biofeedback can reduce chronic pain, so can mindfulness training and yoga, Miles O’Brien reports.

Chronic pain can be debilitating, but there are ways to help your mind and body manage pain. Matthew Schumann, licensed psychologist with Mayo Clinic, discusses factors that impact pain, and reviews evidence-based self-management strategies to reduce pain.

Central sensitization is a condition that affects many people with chronic pain. The person feels like brain fog, difficulty to concentrate, poor memory, low mood, low energy, fatigue and has pain in many parts of their body. This is common in people with fibromyalgia and complex regional pain syndrome. Central sensitization is also present in people who have opioid-induced hyperalgesia.
The treatments involve mind-body therapies and movement (exercises).

Drug-related deaths among people 18 to 34 soared 108% between 2007 and 2017, while alcohol deaths were up 69% and suicides increased 35%, according to an analysis of the latest federal data by the non-profit Trust for America’s Health and Well Being Trust.

Maine has one of the highest overdose rates in the country, with a record 378 deaths in 2016. And the state has responded aggressively, by passing one of the nation’s strictest laws on prescribing opioids. It caps most patients at 100 milligrams of morphine, or its equivalent, per day.

But the law has had unintended consequences. Hundreds of chronic pain sufferers are now finding it impossible to access their typical dosage. After some Mainers pushed back on these restrictions, lawmakers said they could qualify for higher doses under the law’s palliative care exemption. Despite these assurances though, many doctors have their own interpretation of palliative care, considering it reserved for symptomatic relief at end of life. And with the medical community often blamed for causing the opioid crisis, patients are struggling to find doctors to give them the medicine they need. VICE News visits chronic pain sufferers in Midcoast Maine and one of the only physicians willing to exempt them from the new law.
